General order form for seat reservation is here. Print this and fill in the form. And take it to ticket counter. Actually you can reserve seats without this form, but with it you can reserve seats easier. Company JR-East made this sheet, but you can use this sheet for other rail, because the sheet consisted of very common items familiar to any rail companies.
